I understand that Revival is a Smoke free and Substance-Free Workplace and conducts drug and alcohol testing of applicants. I further understand that I must sign a “Pre-Employment Drug/Alcohol Testing and Consent Form”. Refusal to sign this form or failing the drug or alcohol test will prohibit me from being employed at Revival Animal Health, and any offer of employment will be withdrawn. I understand that a copy of Revival Animal Health’s substance abuse/testing policy is available upon request.
